What the Pharmacist, Ambulatory will need:
- BS degree in Pharmacy.
- Preferred: Advanced degrees (i.e., Doctor of Pharmacy)
- Licensed to practice in the State of Michigan.
- Preferred: PGY1 or PGY2 ASHP accredited residency
- Preferred: 2-3 years in hospital or ambulatory setting
- Manage patients on anticoagulants by interpreting treatment goal and adjusting anticoagulants as necessary in Mercy Health's anticoagulation clinic.
- Collaborate with other healthcare providers as well as providing direct patient care.
- Meet with patients individually to provide extensive drug and disease state education targeting complex disease states such as diabetes, hypertension, dyslipidemia, asthma, COPD, tobacco dependence, and osteoporosis.
- Work with patients to improve adherence and evaluate medication regimens.
- Assist in supervision of pharmacy technicians
