You’ll help individuals regain independence and improve their quality of life through personalized care plans and therapeutic interventions.

What You Will Do
- Assess clients’ functional status and develop individualized treatment plans in collaboration with physicians.
- Deliver preventive, therapeutic, and rehabilitative services in accordance with the plan of care.
- Educate clients and families on safety, health promotion, and use of prosthetic/orthotic devices.
- Supervise Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs).
- Coordinate care with interdisciplinary teams and participate in case conferences.
- Document all care activities accurately using point-of-care systems.
- Graduate of an accredited Occupational Therapy program.
- NBCOT certification and state licensure/registration.
- Minimum one year of experience or completion of a 6–8 week preceptorship.
- Valid driver’s license and reliable transportation.
- Home health experience preferred.
- Commitment to ethical standards and continuous learning.
