Position Highlights
- Effectively and efficiently plans visit content and schedule based on client capabilities, condition, preferences (when possible), physician orders and available resources.
- Maintains a working knowledge of applicable Federal. State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and adheres in a manner that reflects honest, ethical, and professional behavior.
- Performs specific treatments and administers medications to implement the medical and nursing care plans.

- Graduate of an accredited or state approved school of practical nursing
- Licensure as a LPN in the State of Michigan
- A minimum of one (1) years of experience in an acute care setting; home care experience preferred.
- Phlebotomy and wound care experience required.
- Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
- Must possess interpersonal skills sufficiently to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors.
- Must have current Driver’s license and reliable transportation to and from worksite.
