Trinity Health at Home is seeking a dedicated and compassionate Medical Social Worker to join our team in a casual/contingent role. This position involves providing consultation and services to homebound individuals, addressing complex social and emotional problems that impede the management of medical regimens.
Key Responsibilities:
- Conduct comprehensive assessments of patients and caregivers to formulate appropriate care plans.
- Develop person-centered plans of care to promote holistic well-being.
- Collaborate with physicians and other health team members to address significant social and emotional factors affecting patients' health.
- Assist patients and families in understanding and following medical recommendations.
- Utilize community resources to help patients resume life in the community or manage disabilities.
- Participate in staff meetings, policy formation, and treatment planning.
- Provide consultative services to Regional Directors and VP.
- Maintain confidentiality of patient, staff, and organizational information.
Minimum Qualifications:
- Master's Degree in Social Work from an accredited school.
- Valid Social Work License as per state regulations.
- Two years of social work experience in a healthcare setting.
- Ability to perform written, verbal, and observational tasks related to the position.
- Professional demeanor and commitment to patient confidentiality.
- Ability to provide services to acutely and chronically ill patients.
