What are the responsibilities and job description for the LPN position at Trinity Health At Home?
Coverage Area: Greater Enfield and Stafford Springs
Schedule: Monday - Friday, 8a- 5p with every 4th weekend and 1-2 holidays per year

Basic Job Function Summary: Provides skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician and assigned by the supervising registered nurse. These nursing activities are limited to those in compliance with the state’s/province’s LP(V)N/RNA Practice Act, any applicable licensure/certification requirement, and agency policies and procedures.
Minimal Qualifications:
- Graduate from an accredited or state approved school of practical nursing
- Current licensure in the state of practice
- Must have a minimum of one (1) year experience in an acute care setting.
- Home healthcare experience preferred.
- Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
- Must have current Driver’s license and reliable transportation to and from work site.
