We are seeking a dedicated and experienced Hospice RN Case Manager to join our team of excellence. The ideal candidate will possess strong nursing assessment skills, skilled nursing care, and case management to those facing end of life. As a Hospice RN Case Manager, you will coordinate comprehensive care plans, advocate for patients’ needs, and ensure seamless communication across multidisciplinary teams to deliver exceptional end-of-life care.

Knowledge and skills required:

· Knowledge of professional nursing practices and principles

· Knowledge in developing nursing care plans

· Knowledge of current methods in pain and symptom control

· Ability to adapt nursing principles and practices to a variety of home settings

· Knowledge of current methods in chronic illness, terminal illness and geriatric nursing

· Excellence in written and oral communication

· Demonstrates excellent interpersonal skills and team working skills

· Adaptable to a wide variety of changing situations

· Computer knowledgeable with keyboarding skills necessary to perform clinical charting on a computer

· Ability to prioritize and organize work effectively and efficiently to accomplish workload in the allotted time.

· Ability to facilitate positive morale with co-workers, clients, families and other professionals

· High degree of professionalism when working with staff, clients and public

· Ability to be confidential with client, staff and business information

Agency Expectations:

· Maintain the vision, mission and values of Triniti Home Health and Hospice

· Remain up-to-date regarding agency policies and procedures and perform all functions in accordance with Triniti Home Health and Hospice policies and procedures

· Provide the highest quality of customer service consistent with Triniti Home Health and Hospice values and standards.

· Maintain always client confidentiality as per the Health Insurance Portability and Accountability Act (HIPAA).

· Develop and demonstrate a set standard of personal expectations for excellence in quality and quantity of work.

· Demonstrate the ability to build relationships and work cooperatively with others, be part of a team, express positive attitudes and expectations of others, build team commitment by promoting good working relationships regardless of personal likes or dislikes, and sustain professional contacts for the purposes of networking within the team and the field.

Essential Functions:

· Attendance

· Provides skilled nursing care under the physician’s orders

· Provides nursing assessment, intervention, and evaluation for each individual client

· Promotes, maintains, and restores health through teaching, counseling and use of appropriate preventative, rehabilitative and /or palliative measures

· Collaborates with the client’s physician regarding the plan of care and arranges for the Face to Face as required.

· Maintains accurate documentation in timely manner

· Participates in case conferences, or IDG meetings

· Case manages home care/hospice needs of clients to provide a safe care plan, using all available community resources

· Delegates personal care or homemaking duties to home health aides and homemakers. Participates in instruction, supervision and evaluation of aides and homemakers

· Assures quality of care is maintained and client’s needs are appropriately met

· Assists with Quality Assurance and Review as requested

· Demonstrates alignment with the Agencies mission, vision, and values

· Position may involve rotation of on-call shifts to provide 24-hour RN availability

· Attends staff meetings and educational programs as required

· Follows State and Federal Regulations for Home Care and Hospice

· Follows all safety policies and procedures

· Other duties as assigned

Reliable Transportation Necessary

Work Environment:

· Requires daily travel by automobile in all weather conditions.

· Occasional exposure to unclean or unsanitary conditions during course of home visits including cats, dogs, smokers.

· Smoke free office.

Physical Demands:

This position involves sitting, driving, standing, walking, repeated bending, squatting, stooping or similar strenuous duties, hand repetitive movements and viewing a digital monitor. This position also involves lifting to 50 lbs. occasionally.

Pay: $40.00 - $46.00 per hour
