Qualifications:
Registered Nurse licensure through State of Minnesota
- Minimum five years experience as Registered Nurse.
- Bachelor’s degree preferred
- Home Health Care experience required
- Management or supervisory experience preferred
- OASIS Certification, or extensive OASIS experience preferred
Accountable To:
Director of Home Health
Primary Objective:
To manage and provide leadership to designated home health personnel. To provide oversight of all patient care services for your demographic territory ensuring that well-coordinated, and high-quality care services are provided utilizing best practices.

Essential Functions:
Attendance for reliability for clients to receive uninterrupted services
- To provide oversight of home health patient care services: assisting intake with coordinating referrals and making patient and personnel assignments and coordinating patient care to assure that patient needs are being assessed and the plan of care is being developed, implemented, and updated according to patient needs.
- Process a variety of clinical workflow in the electronic medical record back-office module.
- Work with intake and care coordinator(s) as needed during intake process by reviewing and approving referral, assigning case manager, and licensed professional to complete admission evaluations and communicate plan to care team.
- Ensure that home health services are consistent, well-coordinated and that high-quality care is rendered, according to agency policy, through review of admission, transfer, resumption of care, recertification, and discharge evaluations, 485, orders, OASIS, and care plans
- Run and analyze payroll and productivity reports to assist with management of overtime and caseload.
- Resolve clinical documentation issues to facilitate billing.
- Participate in care coordination/intake meetings with focus on managing caseloads and census, transfers and resumption of care, and increasing census.
- Provide coaching and education to staff as needed, and performance reviews as needed.
- Organize staff meetings and regular interdisciplinary team meetings/care conferences.
- Participate in Quality Assurance and Performance Improvement (QAPI) and Process Improvement Plans
- Follow State and Federal Regulations for the provision of Home Care services.
- Participate in the agency leadership team to institute best practices. Attend leadership meetings/conferences as applicable to manager role.
- Participate in financial oversight and assistance with budgeting of the home health services delivered.
- Provide coverage in the absence of Director of Home Health and/or other Clinical Managers.
- Participate in marketing the home health agency by developing and maintaining a positive working relationship with community professionals.
- Hire, orient, supervise, provide performance reviews and disciplinary action, and evaluate home health staff (RN, LPN)
- Manage schedules of nursing staff (work hours, days, PTO) and develop weekend/holiday nursing schedule.
- This position also requires weekend and holiday rotation to provide availability of a home health manager to address home health issues/concerns workflow needs.

Reliable Transportation Necessary
Work Environment:
Most work will be completed in a smoke free office. May occasionally be exposed to unclean or unsanitary conditions during course of home visits or co-visits with field staff - including cats, dogs, and smokers.
Physical Demands:
This position does require prolonged sitting, repetitive finger / hand movements and viewing a digital monitor. Occasional standing, driving, bending, walking, stairs, squatting, stooping, or similar strenuous duties. Lifting requirements would be up to 50 lbs. occasionally.
Pay: $85,250.00 - $106,000.00 per year
