Job Summary: Are you passionate about delivering comprehensive, patient-centered care in a collaborative environment? Trillium Health is seeking an experienced Physician Assistant or Nurse Practitioner to join our innovative, mission-driven team. Reporting to the Director, Advanced Practice Providers and working under the direction of the Chief Medical Officer, you’ll provide high-quality primary and/or specialty care to a diverse patient population. Your role will include direct patient care, health education, and active participation in our interdisciplinary team.
Duties/Responsibilities:
- Develop and maintain a practice agreement with a collaborating physician in accordance with NYS guidelines.
- Submit proof of practice agreement to the State Education Department within 90 days of hire.
- Deliver patient care including histories, physical exams, urgent and routine visits, office procedures, and test result reviews.
- Educate patients, families, and healthcare staff on a variety of health topics, including nutrition, stress management, and disease prevention.
- Participate in case management meetings and supervise clinical interventions by RN staff.
- Engage in continuing education, including primary care and HIV/AIDS care, and participate in peer and chart reviews.
- Maintain professional credentials and practice in accordance with ANA Scope and Standards of Practice.

Education and Experience:
- Minimum 3 years’ experience as a board-certified Nurse Practitioner or Physician Assistant.
- National certification by ANA or an equivalent body.
- Experience in community-based healthcare settings is preferred.
- Proficiency with Epic EMR, strong interpersonal and communication skills, and computer literacy.
Physical Requirements:
- Ability to stand and walk for prolonged periods during the workday.
- Lift and carry up to 50 pounds occasionally, including assisting with patient transfers and handling equipment.
- Frequent use of hands and fingers for medical procedures and computer work.
- Regular bending, kneeling, and reaching required.
- Visual and auditory acuity to assess patient conditions and respond to alarms.
- Ability to handle repetitive motions and manage multiple tasks during extended shifts.
