Full Time Night Shift - Hospice Aide II

This full-time evening position supports daily operations and patient care coordination in Hospice of Cincinnati’s Central Inpatient Unit in Blue Ash. Working 10:00 AM–10:30 PM, the role includes two weekly shifts as a Unit Coordinator and one weekly shift as an Aide, assisting with patient flow, call response, interdisciplinary communication, and compassionate service delivery. The position requires strong communication skills, basic computer proficiency, flexibility, and includes shift differential and weekend premium incentive pay.

Job Overview

The Hospice Aide II position is an advanced position that provides direct patient care to Hospice of Cincinnati patients under the direction of a nurse. This position provides assistance and/or performs basic activities of daily living including bathing, feeding, assistance with ambulation, etc. and the hospice aide will serve as provider of companionship and emotional support, even during times of crisis. This position is responsible for knowing safety policies and following approved procedures. In addition to the hospice aide responsibilities listed above, the Hospice Aide II role will also include one of the following responsibilities: A. Unit Coordinator for assigned unit and shift. May perform the duties as listed for both Hospice Aide and Unit Coordinator roles during each shift as directed. B. Cross-trained to function and work in more than one setting or level of hospice care settings, including inpatient hospice units, home care/long term care/assisted living, and continuous care. C. Precepting and mentorship support: assist with precepting, orienting, and mentoring new hospice aides.

Job Requirements

High School Diploma or GED 2 years high school education in general studies (Required)

2 - 5 Years Of Previous Healthcare Experience (Required)

Age 17 with work permit

Reading and math ability appropriate to the position. Multi-line phone skills, familiar with patient call systems and pagers.

Basic Life Support (BLS) within 30 Days Required

Job Responsibilities

Communicates effectively with others. Shows respect to patients, families, and team members, inside and outside the facility, and directs and facilitates unit flow Reports patient related information in a timely manner, and reports problems in completing assignment/tasks in a timely manner. Documents observations and tasks performed. Works in collaboration with all members of the health care team. Utilizes chain of command appropriately. Answers phone and patient calls. Prioritizes work and responds appropriately to emergency situations. Uses emergency systems, policies, and procedures. Performs computer down time procedures according to unit/TriHealth guidelines.

Maintains a safe environment. Follows patient fall prevention guidelines, reports safety hazards to appropriate personnel. Completes safety event report as necessary. Complies with Infection Control policies. Demonstrates knowledge of safety/emergency policies and procedures and appropriately corrects any identified hazards. Places emergency calls timely as directed. Communicates appropriately and effectively with internal and external customers (i.e. staff, patients, families, other departments, etc.). Relays accurate and complete information. Maintains confidentiality. Takes an active role in maintaining patient satisfaction (i.e. way-finding, quick response to call lights and phones, etc.).

Documents accurately in the medical record as per unit/Hospice of Cincinnati guidelines. Maintains the medical record when performing unit coordinator responsibilities. Performs computer downtime procedures according to guidelines. Maintains proficient knowledge of computer applications specific to their unit/job. Provides other clerical support to unit as needed and directed by charge nurse, lead nurse, or manager.

Performs and documents basic patient care tasks under the direct supervision of a nurse. Maintains competency of tasks in which the hospice aide is trained. Completes assigned tasks within required timeframe. Seeks clarification from the nurse when in doubt about how to proceed. Maintains proficient knowledge of computer applications specific to unit/job/ (i.e. maintaining census, accurate scanning of documents, accurate transcription of physician orders). Is knowledgeable of the function/operation of equipment specific to the role on the unit.

Performs supply, equipment, and environmental-related activities as required. Recognizes and reports problems with supply/equipment availability in a timely manner. Reports and follows through per unit standard concerning equipment that is broken or malfunctioning. Assures equipment/supplies are readily available for use by next person. Is knowledgeable of the function, operation, and maintenance of equipment specific to their role on the unit. Demonstrates/maintains knowledge of organizational and unit changes. Supports organizational and unit changes.

May serve as Unit Coordinator for assigned unit and shift as directed. Responsibilities include clerical duties, including accurate data entry into the electronic medical record. Communicates information through various telecommunication devices with respect and compassion to patients and families. May coordinate patient flow. May provide hospice aide services across multiple settings or levels of care, which may include Inpatient Hospice units, home care/long term care/assisted living, and continuous care. Adapts personal care delivery to align with the environment and setting, patient acuity, and level of care requirements while maintaining hospice specific standards of dignity, comfort, and safety. Demonstrates proficiency in setting-specific workflows, documentation standards, and safety protocols unique to each setting. Maintains flexibility in scheduling and assignments to meet patient and organizational needs. May assist with precepting and mentoring new hospice aide residents or hospice aide I under the direction of a nurse and/or Hospice Aide III. Provides role modeling of hospice aide standards, patient dignity, and organizational policies during orientation and onboarding experiences. Supports new hospice aides by reinforcing proper performance of personal care tasks, safety practices, and hospice philosophy. Offers real-time guidance and constructive feedback related to workflow, professional conduct, and documentation expectations. Communicates observations, concerns, or training needs to the nurse or Hospice Aide III as appropriate.
