Position Overview
We are seeking a highly organized and patient-focused Medical Assistant to support physicians and care teams using a digital care coordination platform within the Universal Patient Link network. This role works directly with provider offices to support patient engagement, care coordination, and day-to-day clinical workflow execution.
This is not a traditional rooming-only Medical Assistant role. This position plays a key role in helping providers manage ongoing patient care, particularly for patients requiring continuous monitoring, follow-up, and coordination across multiple touchpoints.
Key Responsibilities
- Support physicians and clinical staff across in-office and remote care workflows
- Assist with patient intake, follow-ups, and communication related to care plans and medications
- Coordinate directly with patients to complete health check-ins, questionnaires, and follow-up actions
- Monitor patient updates including symptoms, vitals, and status changes; escalate concerns as needed
- Facilitate continuity of care for patients transitioning between care settings
- Track and document patient interactions, care activities, and follow-ups accurately
- Maintain organized patient records and ensure timely provider communication
- Support medication management workflows including reminders and adherence tracking
- Collaborate with internal teams and provider offices to ensure smooth care coordination
- Ensure all activities are compliant with HIPAA and patient privacy standards
Qualifications
- Certified Medical Assistant or equivalent clinical experience
- 2 years in a physician office, outpatient clinic, or care coordination environment
- Strong understanding of clinical workflows and provider support
- Comfortable using healthcare software systems, patient portals, or care platforms
- Strong communication skills with both patients and providers
- Highly organized with strong attention to detail
Preferred Experience
- Chronic care, remote patient monitoring, or care coordination programs
- Medicare-based care models or extended care workflows
- Managing high patient volumes with structured follow-up processes
