JOB DESCRIPTION:
The Nurse Practitioner collaborates with a physician to provide primary care services. They possess advanced skills in physical diagnosis, psycho-social assessment, and management of health-illness needs. Their role involves integrating health maintenance, disease prevention, physical diagnosis, and treatment of common episodic and chronic problems in primary care, while equally emphasizing health education and management. It's important to note that Nurse Practitioners do not hold supervisory responsibilities.
DUTIES
- Administer Treatment as needed.
- Chronic care management.
- Establish specialty medical referral systems.
- Issue prescriptions and immunizations.
- Monitor health conditions.
- Recommend hospitalizations as appropriate.
- Conduct follow-up care.
- Prevention planning and intervention.
- Early identification and diagnosis.
- Provide basic medical care.
- Medical promotion, prevention, and education.
- Collaborate with Substance Abuse Counselors and Center Mental Health Counselors to prescribe medications, schedule II medications, and discuss long-term medication management.
- Advise and assist the health education instructor in the development and implementation of the Medical Health Education Program.
EDUCATION AND QUALIFICATION REQUIREMENTS
- Must have an active unencumbered/unrestricted Nurse Practitioner’s License to practice independently in California.
- Must Have 2 years of a minimum of acute experience in a Nurse Practitioner role.
- Must have the authority to prescribe Schedule II controlled substances (DEA CERTIFICATION).
- Must hold a master’s degree, post-graduate certificate, or doctorate from a nationally accredited advanced practice nursing program recognized by the United States Department of Education (USDE)
- Must be nationally certified by a certifying agency representing your APRN role.
- Must have experience working with geriatric patients.

COMPENSATION
- $134,000-167,000 Annually Bonus Equity
