Tri-State LifeCare Registered Nurse
The TSLC RN is responsible for conducting patient assessments, enrolling patients in Tri-State LifeCare, following the plan of care, providing treatment and working in conjunction with the members of the team in order to ensure comfort and quality of life. This position is responsible for scheduling staff and patients.
Essential Functions:
- Performs assessment of assigned patients and derives nursing diagnoses based on assessment data.
- Plans nursing interventions in collaboration with the interdisciplinary team to resolve current problems or prevent potential problems.
- Provides routine nursing visits and personal care while following the plan of care:
- Provides comfort oriented physical care to patients.
- Provides and documents teaching to patients and/or families to assist in the patient's comfort and records the response of the family and/ or family
- Practices and role models universal precautions in all contacts with patients
- Demonstrates the physical care needed for the patients, family or Hospice Aide
- Promotes working in a team environment.
- Proficiency with scheduling software
- Provides scheduling for staff and patients.
- A problem-solving mindset and the ability to stay calm under pressure
- Performs other work duties as assigned.
- Possesses the interpersonal skills that promote interdisciplinary collaboration and effective communication.
- Strong organizational skills
- Excellent communication skills-both written and verbal-with a warm, professional tone.
Education and Work Experience:
- Minimum of 1-year patient care experience as a registered nurse.
- Previous experience in scheduling, staffing coordination.
- Valid driver's license, automobile insurance, and reliable transportation.
Certification and Licenses:
Licensure in West Virginia and Ohio
