Job Title: Licensed Practical Nurse (LPN)
Schedule: 2–3 days per week, including every other weekend and holiday rotation
Position Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in a 38-bed assisted living community. The LPN will play a key role in delivering high-quality care, ensuring residents’ comfort, safety, and well-being.
Key Responsibilities:
- Administer medications and treatments as prescribed
- Assist residents with activities of daily living (ADLs)
- Monitor and document residents’ health status and maintain accurate records
- Observe residents and report changes in condition to the appropriate healthcare team members
- Educate residents and families on care plans, medications, and health changes
- Maintain a safe, clean, and comfortable living environment
- Follow and implement infection control and safety protocols
Qualifications:
- Graduate of an accredited Practical Nursing program
- Current LPN license in the state of Illinois
- CPR/BLS certification required
- Strong communication, organizational, and clinical skills
- Ability to work independently and collaboratively within a healthcare team
