We're growing and hiring! We are seeking a compassionate, organized, and dependable Full-time Bachelor of Social Work (BSW) Volunteer Coordinator to join our interdisciplinary Home Health & Hospice team.
This unique dual-role position combines volunteer coordination with hospice social work support. The BSW Volunteer Coordinator will oversee TREK Healthcare’s hospice volunteer program, including recruiting, training, scheduling, and supporting volunteers to ensure meaningful connections with patients and families. Additionally, this role will provide support to our Medical Social Worker (MSW) in assisting hospice patients and families with psychosocial needs, resources, and emotional support.
The BSW Volunteer Coordinator is an essential member of the Interdisciplinary Group (IDG) and works collaboratively with nurses, aides, physical therapists, chaplains, volunteers, and other team members to support the physical, emotional, and spiritual needs of hospice patients.
This role requires strong communication skills, compassion for individuals facing end-of-life experiences, and the ability to build meaningful relationships with patients, families, and community volunteers.
Key Responsibilities
Volunteer Coordination
-Recruit, onboard, train, and support hospice volunteers in accordance with hospice regulations and organizational policies
-Coordinate volunteer assignments to ensure patients receive meaningful companionship and support
-Match volunteers with patients based on patient needs, interests, availability, and volunteer strengths
-Maintain volunteer schedules, documentation, and required records
-Provide ongoing communication, support, and recognition to hospice volunteers
-Monitor volunteer involvement and ensure compliance with hospice volunteer requirements
-Collaborate with the hospice team to identify opportunities for volunteer involvement
Hospice Social Work Support
-Assist the MSW with providing emotional and psychosocial support to hospice patients and families
-Assist with identifying patient and family needs and connecting them with appropriate community resources
-Participate in patient visits alongside the MSW as appropriate
-Support bereavement activities and follow-up needs
-Assist with documentation and care coordination related to social work services
-Communicate respectfully and compassionately with patients, families, caregivers, and the interdisciplinary team
-Maintain patient confidentiality and ethical standards at all times
Qualifications
Required
-Bachelor’s degree in Social Work (BSW) from an accredited program
-Strong organizational skills with the ability to coordinate multiple schedules and responsibilities
-Ability to work independently in the community and patient home settings
-Valid driver’s license and reliable, insured vehicle
-Current CPR certification
Preferred
-Hospice, home health, healthcare, or social service experience
-Experience working with volunteers or coordinating community programs
-Knowledge of hospice philosophy and end-of-life care
