Schedule: Part-time (2 days/week to start, increasing to 3–4 days/week)
Compensation: $70/hr annual productivity bonuses based on profitability

Job Overview:
We are seeking an experienced, compassionate Nurse Practitioner with a background in hormone replacement therapy to join our team. The ideal candidate is functional medicine-minded, holistic in approach, and enthusiastic about helping patients achieve their optimal health and wellness. Experience with eClinicalWorks EMR is preferred.
Responsibilities:
- Conduct comprehensive patient assessments, including hormonal, metabolic, and wellness evaluations.
- Develop and manage personalized hormone replacement and wellness treatment plans.
- Provide telehealth and in-person consultations for hormone optimization, weight management, and overall wellness.
- Collaborate with the care team to implement functional medicine-based protocols, including peptides (Seeds protocols).
- Maintain accurate documentation and patient records in eClinicalWorks EMR.
- Educate and empower patients to make sustainable lifestyle and wellness changes.
Qualifications:
- Active Nurse Practitioner license with clinical experience in hormone replacement therapy required.
- Functional medicine or holistic wellness background strongly preferred.
- Experience with eClinicalWorks EMR preferred.
- Excellent patient communication and care coordination skills.
- Passionate about helping patients optimize health, hormones, and wellness.
