RESPONSIBILITIES
- Performs duties and health services consistent with education, training, licensure and certification, consistent with treatment plans outlined for patients by providers.
- Serves as a member the medical services care team.
- Develops mutually agreed upon health care plan(s) with patients that address lifestyle changes and work closely with behavioral health team for support and assistance in this process as needed.
- Tracks the need for routine health maintenance screenings and chronic disease management follow-up and advises the practitioner of tests needed.
- Obtains health history information and orders laboratory studies according to Clinic protocols.
- Provides health education to patients as part of the clinical team, especially for chronic disease management and medication safety.
- Provides telephone triage and follow-up calls, assessing patient’s needs and directing them to appropriate care in appropriate timeframe.
- Tracks ordered diagnostic tests and ensures ordering practitioner is aware of results.
- Provides anticipatory health maintenance guidance.
- Administers immunizations and other injections as ordered by practitioners.
- Collects and prepares lab specimens as may be directed. Performs basic laboratory tests including blood draws.
- Uses the electronic health record as appropriate to the role of the RN, LPN.
QUALIFICATIONS
- Current license as Registered Nurse or Licensed Practical Nurse in the State of Michigan.
- Bachelor’s Degree in Nursing preferred.
- Current BLS Certification and ability to use AED.
- Previous ambulatory primary care experience; previous case management experience preferred.
- Prior experience and proven ability to work as a team member; prior experience and proven ability to work with the public.
- Excellent interpersonal skills; excellent organization skills; ability to multi-task.
- Ability to relate well with diverse populations of varying socio-economic backgrounds.
