Transitions At Home is currently seeking a compassionate and dependable PRN Caregiver to assist clients in Lyndon Station with Cooking, Bathing, Adls, and Transfers. Experience in Catheters and Ostomy Care a plus, but willing to train the right person.

Every client has unique needs, and your responsibilities may include Companionship and social engagement, Light housekeeping , Personal care assistance, Meal preparation, Medication reminders, Running errands, Pet Care and Providing a friendly face and brightening someone's day

What does a Transitions at Home Caregiver do?

An Caregiver performs services for the clients as necessary to maintain the personal care and comfort of the client while in their place of residence. This is performed in accordance with the plan of care under the direction and supervision of the NM Manager.

Hours: 10 Hours a Week

• Monday through Friday

Qualifications / Job Requirements:

• Must be a minimum of 18 years of age
• One year of Caregiving experience preferred
