The Remote Patient Success Manager will provide essential administrative and operational support in a healthcare environment, ensuring smooth and effective communication between providers, patients, and the community. This role is provider-driven, focusing on maintaining strong relationships with healthcare professionals, collaborating with community resources, and enhancing patient care by creating a seamless experience for all stakeholders.
Key Responsibilities:
Provider Liaison:

Serve as the primary point of contact between healthcare providers and administrative teams.

Assist providers with patient data management, and ensure communications are handled within a timely manner.

Support providers in obtaining patient information and handling any issues related to care coordination.

Ensure all medical documentation is accurate and up-to-date.

Calls in Lab and Radiology results
Community Engagement:

Act as a liaison between the organization and community resources, fostering strong relationships.

Develop and maintain communication with local healthcare providers, organizations, and services that benefit patients.

Participate in initiatives designed to strengthen the organization's role in the local community.

Foster relationships with home health liaisons servicing assigned buildings.
Patient-Centered Support:
-
Provide administrative support for patient inquiries, ensuring a compassionate, patient-centered approach.

Ensure patient privacy and confidentiality at all times in accordance with healthcare regulations.

Assist in managing patient records and any follow-up care coordination.

Responsible for appointment reminder calls 72 hours prior to every appointment

Ensuring every new patient receives an initial care plan call to include same page invitation.

Complete care plan calls on specific cadence
-
Facilitate communication between patients and healthcare providers to ensure seamless care delivery.

Assist in forms pertaining to transitions of care (discharges from SNF to ALF, ALF-memory care, ALF-PCH, etc)

During follow-up call requests if refills are needed and send email with requests to Rx team
Operational Support:

Perform general administrative duties such as document management, data entry, and maintaining accurate records.

Track and report patient feedback to improve overall patient satisfaction.
Compliance & Quality Assurance:

Stay updated on regulatory guidelines and ensure all back-office functions comply with relevant laws and standards (HIPAA, etc.).

Contribute to quality improvement initiatives focused on patient care and operational efficiency.

Qualifications:-
Must reside in Georgia

Associates degree
-
Licensed Practical Nurse (LPN) license in the state of GA

Strong interpersonal skills and ability to effectively communicate with healthcare providers, community members, and patients.

Exceptional organizational skills with an ability to multitask and prioritize.

Knowledge of healthcare regulations, HIPAA, and patient privacy laws.

Proficiency in using office software (Microsoft Office, Google Suite, etc.) and healthcare management software.
