As a Case Manager, you'll lead the way in person-centered planning, guiding clients with empathy from their first assessment through each annual milestone or anytime their needs change.
This role takes you beyond the office and into the community! You'll complete home visits, engage directly with consumers where they live, and connect them to essential community resources that empower their growth and stability.

Responsibilities
- Conduct initial and annual psychosocial assessments to understand each consumer’s unique strengths and needs.
- Develop and update individualized, person-centered plans—adjusting them annually or whenever a consumer’s level of need changes.
- Complete home visits, hospital visits, and community outreach to support consumer’s in their environments.
- Deliver timely mental health crisis intervention services to ensure safety and stabilize immediate concerns
- Link consumers to vital community resources and internal services that promote independence, wellness, and stability.
- Use motivational interviewing, screening tools, and treatment matching protocols to guide effective care.
- Maintain accurate documentation and time management skills
- Completes referral forms, clinical and legal documentation necessary to obtain community based services for clients as well as collaboration and communication with other SA/MH community partners
