Responsibilities
- Assess the needs of individuals and families, including their physical, emotional, financial, and social circumstances
- Engage in preventive efforts to promote health and wellness within communities.
- Connect clients with community resources and support services that can address their social and economic needs, such as housing assistance, financial aid programs, transportation services, and support groups.
- Collaborate with healthcare professionals, including doctors, nurses, therapists, and case managers, to develop comprehensive care plans that address the unique needs of each individual.
- Advocates for clients within the healthcare system, ensuring they receive appropriate care and support
Qualifications
- Active Social Worker License
- 2 years of Behavioral Health experience
- 2 years of Case Management experience
