Total Care Services, Inc. (TCS) is seeking a Russian/English bilingual Case Manager for our Adult Medical Day Care Services (MDCSW) program. In this role, you will support individuals with complex medical, social, and functional needs by coordinating services and advocating for access to quality care. The Case Manager is instrumental in helping participants remain safely supported in community-based settings, promoting independence and enhancing overall quality of life.

Compensation & Benefits

• Base Salary: $60,000
• Quarterly Bonus: Up to $1,250 based on performance

About the Role

As a Case Manager – Medical Day Care Services, you will provide person-centered case management to individuals (age 16 ) who meet nursing facility level of care but choose to live in the community.

Key Responsibilities

• Conduct assessments using Person-Centered Planning (PCP)
• Develop and manage individualized service plans
• Coordinate Adult Medical Day Care and community-based services
• Document all service coordination activities in LTSS Maryland daily
• Conduct in-person client visits
• Monitor service effectiveness and client outcomes
• Ensure compliance with program and regulatory requirements
• Maintain accurate and timely documentation

Qualifications

Required:

• Bachelor’s degree in Human Services or related field (or approved equivalent)
• Some experience in case management or service coordination
• Strong communication, documentation, and organizational skills
• Valid driver’s license, reliable transportation, and safe driving record
• Ability to travel locally throughout Maryland
• Must be at least 21 years old
• Ability to pass background checks
• Authorized to work in the United States

Preferred:

• Experience with Adult Medical Day Care or Medicaid Waiver programs
• Familiarity with LTSS Maryland system
• Experience working with individuals with complex medical or behavioral needs

Schedule

• Full-time, Monday through Friday
• Flexible based on client needs
