Overview
We are seeking a dynamic and compassionate Bilingual Case Manager and Latino/Hispanic Outreach Coordinator to join our dedicated team. A few Duties

  • Manage casework for clients by assessing needs, developing personalized plans, and connecting individuals to appropriate services
  • Lead outreach initiatives targeted at Latino/Hispanic communities through community events and 1:1 meetings. Translation for community members.
  • Provide translation and interpretation support to ensure clear and effective communication between staff and clients.
  • Conduct interviews with potential participants, assess their eligibility and determine the most appropriate site to meet each client’s individual needs.
  • Coordinate and facilitate tours of CADS for potential participants.
  • Assist clients as needed, including helping them schedule appointments when they are unsure or unable to do so independently.
  • Identifying and implementing new, engaging experiences for clients both within the program and in the community to promote social interaction, active participation and meaningful engagement.
  • Collaborate with clients and/or caretakers to plan, develop and implement Person Centered Plan on a monthly, yearly, or as-needed basis, establishing new goals as directed by clients.
  • Conduct regular staff meetings to review and update client needs, ensuring that all team members are informed on current needs and service requirements.
  • Oversee client progress and maintain up-to-date client database records, ensuring accurate documentation of progress and any changes.
  • Collaborate with Administration, Program Coordinator and Direct Support Staff to review progress and ensure goals are consistently met.
  • Maintain consistent communication with organizations such as Managed Long Term Care Agency and Case Managers to provide updates or report changes regarding clients and their services, ensuring all communications are accurately documented.
  • Maintain participant records by preparing monthly case notes, documenting activities, tracking changes, and updating prior issues to ensure accurate and current information.
  • Lead therapeutic and recreational group activities at the center and in the community to enhance cross-cultural understanding.
  • Promote the agency’s program within the community by attending local events and delivering engaging public presentations as assigned.
  • Ensure the accurate completion and timely submission of all required documentation and reports.
  • Continuously participate in seminars, workshops and online training programs focused on Case Management, Alzheimer’s/Dementia care, Person Centered Planning along with other state-mandated trainings as required.
  • Proactively shows initiative by taking on additional responsibilities to support team and organizational goals.
  • Maintain ongoing collaboration with team members and supervisors to achieve positive outcomes for all clients, while fostering open and effective communication with staff.
  • Strong planning, prioritization and time management skills are essential for success in this role. This includes setting clear measurable goals aligned with team and organizational objectives.
  • Demonstrate strong ability to work independently.
  • Reliable attendance is essential, to provide consistent support to clients and meet program goals.

Requirements

  • Fluency in both English and Spanish is essential for effective communication with diverse populations
  • Minimum-associate degree in related Human Service field.
  • Minimum-two years’ experience providing case management support for elderly clients.
  • Must successfully complete criminal background checks prior to placement.
  • Must successfully complete health assessment prior to placement.
  • Reliable transportation is required to conduct outreach throughout the county.
  • Reliable attendance is essential in this role, to provide consistent support to clients and meet program goals.
