What are the responsibilities and job description for the Intake Coordinator position at TLC Child and Family Services?
Transitioning to adulthood is hard for anyone, but can be especially challenging for our most vulnerable current and former foster youth. TLC provides comprehensive mobile case management including employment, health and wellness, education, finance management, accessing resources, and navigating the ins-and-outs of independent living. We help youth (ages 16-25) to rise up from foster care successfully by providing a safe housing environment where they can learn and practice the life skills necessary to work on self-identified life goals.
TLC is currently recruiting for a 0.625 FTE (25 hours per week) Intake Coordinator. This position will be onsite, and remote/hybrid work is not available.
Position Summary:
Under the supervision of, and reportable to, the Director of Transition Age Youth (TAY) Housing Programs, this position manages the intake process for TLC’s Transition Age Youth Housing Programs including THPP and THP-Plus and supports the Program Director in management of administrative and procedural aspects of the THP programs.
Essential Functions:
Intake and Admissions:
- Ensure census goals for THP programs are maintained in alignment with contacts.
- Answer inquiries from outside entities to provide program information and placement availability.
- Communicate and meet with placement workers and other service providers throughout the intake process.
- Review program application packets and collect missing information.
- Complete thorough assessment through document reviews and applicant interviews.
- Arrange and lead THP housing tours and pre-screening meetings.
- Organize intake and move-in date.
- Complete the administrative and documentation process for all THP intakes.
- Maintain the THP-Plus waiting list and communication regarding inquiries and referrals.
- Manage inquiries from unsheltered Transition Age Youth and other service providers.
- Implement presentations to the community to spread awareness of the THP programs.
- Conduct informational meetings with County representatives.
- Complete exit interviews with participants upon discharge from programs.
Program Support:
- Ensures efficient integration of the THP-related programs into all aspects of TLC’s business, as appropriate.
- Manage collection and submission of data for THP-Plus.
- Screen staff applicants including reviewing resumes and initial phone interviews.
- Complete staff applicant reference checks.
- Assist in public relations with referring agencies and counties.
- Assist with review of staff documentation.

Qualifications:
- Bachelor’s Degree in Social Work or a related field preferred.
- Experience working with adolescents and/or young adults, such as residential treatment, transitional housing, foster care or childcare. Applicants will be evaluated first on their experience in youth and family work and secondarily in experience in any related field.
- Employment history that shows progression of taking on greater responsibility.
- Obtain and maintain fingerprint, government and agency required background clearances, without additional exemptions.
- Ability to pass a pre-employment physical exam, TB and drug screen.
- Valid California driver’s license with an acceptable driving record and auto insurance showing employee as an insured driver on the policy.
- All employees will be required to familiarize themselves with a variety of written material that coincides with TLC Child and Family Services philosophic orientation toward youth and their treatment needs.

Wage Range: $25.24 – $33.92 depending on years of experience. The salary range for this role may vary above and below the posted range, as determined by experience and licensure. Compensation takes into account several factors, including, but not limited to, a candidate’s experience, education, skills, licensure and certifications, departmental equity, training, and organizational needs. Posted wage ranges are reflective of a full-time/40 hour week position.
