Job Description
The Transition Job Coach supports students ages 18β22 with disabilities in community-based and school-based vocational placements. The Job Coach provides direct on-site coaching, skill training, and fading supports to help students build work readiness, job competence, and independence aligned with IEP transition goals.
Required Experience
Experience working with individuals with disabilities, preferably transition-age students
Experience in vocational training, job coaching, or community-based instruction
Experience implementing behavior and support plans
Experience collecting performance data and documenting progress
Experience supporting independence and adaptive skill development
Core Responsibilities
Provide on-site job coaching at student work placements
Teach and reinforce job tasks using task analysis and systematic instruction
Support development of:
Work habits and stamina
Appropriate workplace behavior
Communication with supervisors and coworkers
Problem-solving and self-advocacy
Implement IEP transition and employment goals
Use prompting and planned fading strategies to increase independence
Collect job performance data and complete coaching logs
Communicate regularly with the Transition Special Educator and program staff
Coordinate with employers and site supervisors under staff direction
Support travel training and community access as assigned
