Job Description
The Transition Program Special Educator provides specially designed instruction and case management for students ages 18–22 receiving special education services focused on post-secondary outcomes, including employment, independent living, community participation, and functional academics.
Required Experience
Experience working with students with moderate to significant disabilities
Experience implementing and managing IEPs
Experience with transition-age instruction (vocational, functional, community-based)
Experience with functional academics and life-skills instruction
Experience with behavior supports and data collection
Core Responsibilities
Serve as IEP case manager for assigned transition-age students
Develop and implement instruction aligned to post-secondary transition goals
Deliver specially designed instruction in:
Employment and vocational skills
Independent living skills
Functional literacy and math
Self-advocacy and self-determination
Coordinate and supervise community-based instruction (CBI) and job site learning
Collaborate with job coaches, related service providers, and agency partners
Conduct transition assessments and progress monitoring
Maintain service logs, progress reports, and compliance documentation
Facilitate student and family participation in transition planning
Required Credentials
Valid State certification in Special Education (secondary or PK–12, as applicable)
Current teaching license in good standing
Safety Care/crisis intervention certification
Background check and fingerprint clearance
