Key Responsibilities
Resident Advocacy: Serve as a trusted advocate, ensuring residents' rights and dignity are upheld.
Individualized Care Planning: Collaborate with interdisciplinary teams to develop and implement comprehensive care plans tailored to each resident's unique needs.
Emotional Support: Provide counseling and support to residents and families during times of transition or crisis.
Resource Coordination: Connect residents and families with community resources, benefits, and support services.
Compliance Excellence: Maintain compliance with all federal, state, and local regulations, including participation in surveys and audits.
Team Leadership: Supervise and inspire the social services team, fostering a culture of compassion and professionalism.
Family Engagement: Build strong relationships with families, offering guidance and open communication to ensure their loved ones’ needs are met.
Qualifications
Bachelor’s or Master’s degree in Social Work or a related field (LCSW preferred).
Minimum 2 of experience in social work, preferably in a Skilled Nursing Facility or healthcare setting.
Strong knowledge of state and federal regulations (e.g., MDS, PASRR, etc.).
Exceptional communication, organizational, and problem-solving skills.
A passion for working with seniors and individuals with complex medical needs.
