What are the responsibilities and job description for the Activity Professional Coordinator position at TIOGA MEDICAL CENTER?
QUALIFICATIONS
Minimum requirements include the following:
High school diploma or equivalent.
Has completed or willing to complete a correspondence course on activity programming or therapeutic recreation.
Completion of the Activity Professional Certification course.
At least 1 year of prior experience working with the elderly preferred.
ESSENTIAL FUNCTIONS
Oversees the Activity Professionals office.
Operates the Tioga Medical Center Long-term Care Facebook page.
Designs activities to promote opportunities for residents to engage in familiar pursuits. Ensure program contains empowerment activities that increase self-respect, maintenance activities that provide a schedule of events that promote physical, cognitive, social and emotional health, and supportive activities that provide stimulation or solace to residents who cannot generally benefit from either maintenance or empowerment activities.
Assists in the planning and implementing of the activities. Conducts in room activities for those residents who benefit from one on one visits.
Creates monthly calendar of activities written in large print and posts in prominent location that is visible to residents and visitors.
Designs activities that include exercise, group, creative, work-type, intellectual, and social.
Designs activities to promote physical, social, and mental well-being of the resident, which provides empowerment, maintenance, and supportive activities.
Ensures activities are offered to Swing Bed patients.
Coordinates resident activities with nursing service, dietary, physical therapy, and community volunteers.
Assists with activity coordination of snacks and dietary needs.
Plans activities for 7 days per week.
Records daily attendance of each resident and charts on activity participation.
Develops an interest sheet for each resident on what they like to do and what they are presently doing and reviews this sheet annually.
Attends care conference meetings weekly.
Coordinates weekly activity meetings.
Assists in maintenance of recreation equipment and operates them.
Assists in completing MDS sections dealing with activities.
Helps promote activity involvement with the community and surrounding area.
Coordination of volunteer activities. Supervises volunteers when present.
Ensures proper supplies are available to activity staff. Reports needed items or supplies to Activity Director.
Assists with resident outings.
Assists in completing initial, quarterly, and annual activity assessments.
Coordinates funeral services in the chapel.
Develops & Maintains the chapel schedule.
Develops & Maintains the beauty shop schedule.
Member of the Quality Assurance program.
Assists with tray preparation.
Attends department and in-service meetings.
May be required to travel to attend work-related meetings and educational opportunities.
Performs all duties or tasks requested by supervisor or the CEO.
ADDITIONAL TASKS
Treats all residents and patients with dignity and respect. Promotes and protects all residents' and patients' rights.
Establishes a culture of compliance by adhering to all facility policies and procedures. Complies with standards of business conduct, and state/federal regulations and guidelines.
Follows appropriate safety and hygiene measures at all times to protect residents, patients, and themselves.
Understands and demonstrates knowledge of infection control practices, Universal Precautions, OSHA Blood Borne Pathogen Standard, and follows compliance with established policies and procedures.
Maintains confidentiality of protected health information, including verbal, written, and electronic communications.
Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel. Reports any retaliation or discrimination to HR or CEO.
Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and/or administrator. Protects residents and patients from abuse and cooperates with all investigations.
Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy.
Participates in all life safety and emergency drills and trainings. Fulfills responsibilities as assigned during implementation or activation of the facility's emergency plan.
Reports work-related injuries and illnesses immediately to supervisor.
Follows established infection control policies and procedures.
As a condition of employment, completes all assigned training and skills competency.

Ability to read, write, speak, and understand the English language.

Must be able to move intermittently throughout the workday.
Meets general health requirements according to facility policy, including medical and physical exams and checking immunity status to various infectious diseases.
Ability to work beyond normal working hours and on weekends and holidays when necessary.
Ability to assist in evacuation of residents and patients during emergency situations.
Ability to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position.
Ability to lift and carry up to 25 pounds.
Ability to sit for prolonged periods of time at a desk while working on a computer.
Must have basic computer skills.
May be subject to slips, falls, burns from equipment, cuts, and/or odors throughout the day, encounter reactions from dust, disinfectants, and other air contaminants.
Subject to exposure to infectious waste, diseases and/or conditions which include AIDS, Coronavirus, Hepatitis B, and Tuberculosis.
May be subject to hostile or emotional residents, patients, family members, visitors, or personnel.
Must be able to follow oral and written instructions.
Works in well-lit, ventilated area.
Subject to frequent interruptions.
Communicates with medical and nursing staff, and other departments.
Subject to call-back during emergency conditions.
