Forensic Housing Mental Health Care Provider (BA level)

The FHARPS Trueblood Mental Health Care Provider (MHCP) is responsible for providing comprehensive, strength-based case management services to Trueblood Class Members housed in the Olympic Health and Recovery Services Trueblood Houses. The MHCP supports participants in achieving housing stability and successful community integration through individualized service planning, coordination of care, and connection to supportive resources. In addition to direct client support, this position is responsible for developing and maintaining relationships with landlords and property managers throughout Thurston and Mason Counties. The MHCP conducts outreach to expand housing opportunities for FHARPS participants, educates housing providers about the program, and advocates for participant placement, helping to build a network of safe, affordable, and supportive housing options.

Salary Range: $27.72 - $36.45 per hour based on education, experience, and parity within the organization. 

Schedule: Monday-Friday 8am-5pm or 8am-4:30pm

This position reports to the Housing Support Program Supervisor

MAJOR RESPONSIBIITIES:

Case Management

• Maintain an active caseload of residents living in the Trueblood Houses and deliver intensive case management services, including conducting needs assessments, developing person centered housing stability plans with the resident, supporting participants in achieving their goals, and connecting residents with services that address social determinants of health.
• Meet with participants in the OHRS Trueblood houses 
• Conduct resident intakes including supportive housing and educate and inform residents about program requirements and expectations to promote housing retention.
• Support residents in developing skills to maintain stable housing, including employment, advocacy, financial education, benefits navigation, gathering essential documents, and accessing community resources.
• Conduct regular check-ins and monitor participant progress, well-being, and engagement, including support for individuals with acute behavioral health presentations.
• Coordinate with behavioral health providers, medical professionals, tribal programs, and other community-based services to streamline referrals and enhance care.
• Build and maintain relationships with community resources, housing agencies, and system partners to support participant needs.
• Facilitate/co-facilitate house meetings and safety procedures training with residents. 
• Participate in internal and external meetings, including Hospital Diversion Department meetings, FHARPS Trueblood Housing staff meetings, and meetings with DSHS and HCA.
• Collaborate with other Trueblood projects (e.g., FPATH, OCRP, FN, CHV) to provide integrated case management services.
• Maintain accurate, timely, and confidential documentation in the EHR and other required systems, including tracking participant progress, interventions, outcomes, and funds distributed.
• Ensure program compliance with Federal, State, County, and agency requirements.
• Contribute to program development, continuous quality improvement, and best practices in housing case management.
• Maintains physical security of confidential materials and assigned equipment. 
• Transport participants as necessary using agency vehicles.
• Will likely enter facilities or institutions, including but not limited to local jails or hospitals. 
• Promote the OHRS mission, values, and best practices in all interactions with residents, stakeholders, and community partners.
• Responsible for maintaining referral, requisition, and vendor forms, and additional documentation for FHARPS Trueblood House residents and program requirements.
• Assists FHARPS Trueblood staff with projects and tasks to support program operations.
• Collaborates, consults, and works under the supervision of the FHARPS Program Supervisor.
• Perform related work, special projects, or other duties as assigned.

Landlord Engagement

• Promote the FHARPS program and build relationships with landlords in Thurston and Mason Counties to expand housing options
• Coordinate outreach with apartment associations, the Board of Realtors, tenant councils, and other partners to recruit landlords for FHARPS and other rental assistance programs.
• Maintain regular communication with landlords and property managers through outreach and property visits.
• Stay current on Housing Choice Voucher, Project-Based Voucher, and THA Special Population program rules.
• Complete the monthly Landlord Outreach Log, contacting at least five landlords or housing types and reporting unit details, eligibility, and costs to the FHARPS Supervisor.
• Update the Global Lease “Occupancy Tracker” as needed.
• Work with the FHARPS Housing Specialist to negotiate leases and coordinate housing placements.
• Maintain a database of available units, including locations, rents, and eligibility criteria.
• Act as a liaison between residents and housing providers to support housing retention.
• Attend community meetings and events to promote landlord engagement.
• Conduct Housing Quality Standards (HQS) pre inspections of units.

EDUCATION AND EXPERIENCE:

• Bachelor’s degree in psychology, social work, or a related area OR 2 years of college coursework that includes behavioral health courses AND 2 years of direct services behavioral health experience supervised by an MHP; or combination of school and experience
• Two (2) years of administrative and/or clinical experience within a social service or a closely related environment preferred.
• Current Washington State Driver's License, proof of insurance, and satisfactory drivers record that meets insurance coverage requirements. 
• Qualifies as an Agency Affiliated Counselor.
• Satisfactory criminal background clearances and excluded party sanctions.
• Must be able to obtain jail clearance.

Preferred Qualifications: 

• Experience in housing, property management, or a related field.
• Knowledge of the local rental market and landlord-tenant laws and regulations. 

SKILLS AND ABILITIES:

• Ability to utilize the Recovery Model, Evidence Based Practices and effectively engage with program participants. 
• Ability to utilize various skills, methodologies, and approaches including motivational interviewing, trauma-informed care, harm reduction approach.
• Ability to navigate and access systems related to, but not limited to, food, cash, and other medical public benefits through Apple Health, DSHS, and Social Security Department.
• Ability to navigate and access systems, resources, and services related to Behavioral Health Services, medical and/or dental care, and basic needs.
• Ability to navigate and access systems, resources, and services related to emergency, transitional, supportive and permanent housing.
• Familiarity and knowledge of the Permanent Supportive Housing model and the Housing First approach, including principles of low-barrier access, harm reduction, and long-term housing stability for individuals with complex needs. 
• Familiarity with trauma-informed care for people with complex needs.
• Familiarity and knowledge of best practices for alcohol/drug abuse treatment and prevention programs, mental health and developmental disabilities programs as well as with local social service agencies.
• Ability to establish and maintain professional boundaries, trusting and supportive relationships as well as provide inspiration and motivation to individuals transitioning toward recovery.
• Knowledge of symptoms, common behavioral and social characteristics, and other manifestations of the behavioral health challenges affecting the population served.
• Knowledge of the various presentations of behavioral health related illnesses the participants may experience.
• Ability to work in high stress, high demand environment while communicating clearly and effectively.
• Ability to write clear and concise clinical documents, reports, and correspondence including documentation of all client interactions utilizing DAP style documentation within 24 business hours.
• Ability to work collaboratively utilizing strong written and verbal communication skills as part of a multidisciplinary team while respectfully managing differences, opposing perspectives, opinions, and interests.
• Ability to remain flexible and adaptable to change while maintaining effective time management skills.
• Knowledge of and ability to recognize compassion fatigue and the need for self-care.
• Strong critical thinking skills, ability to conceptualize and implement creative solutions.
• Promote and champion the values of partnership, teamwork, compassion and integrity in all dealings within and outside of OHRS.

WORKING CONDITIONS:

• Primarily works in the community including jails, residential facilities, individual’s homes, and in an office environment.
• Regular use of fleet vehicles and/or personal vehicles may be required for service provision in the region including transportation of participants in fleet vehicles.
• May require availability to work flexible hours.
• Dress is professional and appropriate for community-based settings.
• Regular, in person, predictable attendance is necessary for participant support and team engagement.
• Moderate physical exertion is required.  May be required to sit, stand, squat, walk short to moderate distances on uneven terrain, lift or move boxes/equipment weighing up to 25 lbs. May be required to walk distances in outdoor community-based setting to include uneven terrain and subject to weather conditions.
