Job Description
This position involves providing general Physical Therapy evaluations and treatments that enhance patients' functional abilities. The role includes overseeing and directing the work of Physical Therapist Assistants and/or other designated staff members.
Responsibilities
- Conduct physical therapy evaluations and treatments for a diverse patient population with various diagnoses and issues.
- Assess, evaluate, identify therapy diagnoses, formulate treatment plans with goals, and implement physical therapy interventions.
- Supervise and modify therapy plans based on patient responses to optimize outcomes.
- Review clinical records and patient case histories.
- Participate in care conferences and meetings while remaining in contact with referring providers and healthcare team members.
- Collaborate with the healthcare team to arrange appropriate medical equipment and establish care plans prior to patient discharge.
- Document patient encounters, treatments, diagnoses, treatment plans, recommendations, and prognoses in compliance with established policies.
- Ensure timely service delivery according to regulatory requirements.
- Work with both inpatient and outpatient populations in various assigned settings.
- Direct the work of support staff, students, or interns.
- Facilitate a healthcare partnership between providers, patients, and their families.
- Develop and implement patient education and therapy programs tailored to patient and family needs.
- Provide clear and concise home exercise programs to maximize rehabilitative potential and ensure understanding through feedback and demonstrations.
- Advise patients and families on methods to promote self-care and independence.
- Assist senior staff with administrative tasks, clinic operations, and specialty/research projects as needed.
- Utilize hospital systems to access patient information and remain informed about hospital directives.
- Participate in the efficient flow of information via email and digital communication platforms as necessary.
- Provide summaries of results, statistics, and metrics as required.
- Assist with the acquisition and fitting of durable medical equipment based on patient needs.
- Communicate with supervisors and/or DME providers to ensure proper measurement, fitting, and delivery of equipment, maintaining appropriate stock levels.
- Maintain clinical space cleanliness and manage supply stocks and clinical equipment.
- Stay updated on billing procedures for reimbursement of services and plan of care concurrence with referring providers.
- Educate staff and inform patients/families about functional limitations, therapeutic exercises, and equipment to achieve desired goals.
- Represent the Rehabilitation Services Department in committee and management duties as needed.
- Provide therapy consultation, presentations, and in-services to healthcare personnel and patient populations as assigned.
- Adhere to regulatory and practice standards set by the American Physical Therapy Association (APTA).
- Follow confidentiality and quality assurance standards, engaging in quality management functions as needed.
- Perform other duties as assigned.
Qualifications
- Entry-level qualifying master’s or doctorate degree in Physical Therapy from an accredited program OR a graduate from an APTA approved program with two years of proven work experience and Continuing Education.
- Active Physical Therapist license.
- Current Basic Life Support certification.
- Valid Driver's license.
Experience
- Minimum of one year of experience in adult outpatient rehabilitation.
Shift Information
Monday to Friday, 8 AM to 5 PM.
