Shift Information

Variable shifts for departmental coverage, typically a five-day work week including weekends as needed.

We are seeking a dedicated Occupational Therapist to provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). The ideal candidate will have a strong background in patient evaluations, treatment planning, and collaborating with interdisciplinary teams to enhance patient outcomes.

• Provide comprehensive rehabilitation services in an SNF and/or ICF setting.


• Conduct evaluations, reassessments, treatments, and various clinical services relevant to licensure and scope of practice.


• Develop and implement individualized treatment plans in collaboration with supervising therapists, physicians, and the interdisciplinary team.


• Utilize evidence-based therapeutic interventions to promote function, independence, communication, mobility, swallowing, and quality of life.


• Educate residents, families, and caregivers on treatment plans and patient progress.


• Ensure accurate, timely, and compliant documentation of patient progress, changes in care plans, and discharges according to facility and regulatory requirements.


• Supervise therapy assistants as required by role and licensure.


• Be available to work on Hawaii State and Federal Holidays as operational needs dictate.

• Minimum of two (2) years of experience in an SNF/ICF setting is preferred.


• Current Basic Life Support (BLS) certification.


• Active State of Hawaii license in good standing appropriate to the discipline by the start of the assignment.

Orientation will be provided to familiarize the new hire with the rehabilitation department, electronic medical records documentation (Net Health Rehab Optima & Epic), and facility workflows.
