Physical Therapist (PT)

Job Summary

This role involves providing physical therapy services primarily in home settings, including assisted living facilities, senior apartments, and board and care homes. The Physical Therapist will work with a varying diagnosis population, excluding pediatrics and hospice, and will contribute to patient care through evaluations and treatment plans.

Position will cover multiple areas, primarily serving patients from various hospitals and community doctors in the Sacramento, CA region.

• Conduct physical therapy treatments in home settings.


• Manage a caseload of approximately 3 to 5 patients daily, including start of care visits and evaluations.


• Collaborate with nursing and rehabilitation teams for comprehensive patient management.


• Supervise up to 2 Physical Therapy Assistants (PTAs)/Certified Occupational Therapy Assistants (COTAs).


• Ensure effective communication with patients and families, offering education and support regarding therapy processes.


• Complete documentation and case management tasks within designated timelines.

• Current Physical Therapy License in California.


• Two years of experience in a healthcare setting required; community/home health experience preferred.


• Basic Life Support (BLS) certification required.


• Experience with Home Health EMR systems, particularly HomeCare Home Base (HCHB), is preferred.


• Must possess a valid driver's license and proof of automobile insurance.


• Strong oral and written communication skills.


• Good organizational skills and ability to work independently in various home environments.

Full-time position, Monday through Friday, with shifts from 8:30 AM to 5:00 PM. Participation in occasional weekend work and holiday coverage may be requested.

New hires will undergo a comprehensive three-week orientation process, including online learning and field precepting, with expectations of independent work by the end of the training period.

Professional attire is required, including business professional clothing and closed-toe shoes. Scrubs are recommended but not provided.
