Travel Home Health Physical Therapist

Role: Travel Home Health Physical Therapist responsible for providing therapy services in patients' homes within the Sacramento region.

• Responsibilities: Assess, develop, and implement individualized treatment plans to improve patient function and manage chronic conditions; collaborate with interdisciplinary teams; complete documentation in electronic medical records; support patient recovery and quality of life.
• Key duties: Travel within the region, potentially float to nearby areas, and deliver high-quality home health therapy services.
  
  

Qualifications & Skills

• Degree in Physical Therapy (Bachelor’s, Master’s, or Doctoral) from an accredited program.
• Active California Physical Therapy license.
• Preferred experience in home health settings and familiarity with electronic documentation systems.
• Ability to obtain BLS certification from AHA or ARC.
