Lead Clinical Pharmacist

Supervise a team of pharmacist(s), technician(s) and other support staff in administering and completing clinical services.  Perform all duties of a staff pharmacist by conducting direct patient care services.  Coordinate and help manage clinical pharmacy patients, programs, and initiatives, including but not limited to; performing professional clinical reviews of clinical protocols needed for patient care,  conducting Medication Therapy Management (MTM), administration of immunizations, Remote Patient Monitoring, Chronic Care Management, Adherence Programs,  Disease State Management services,  Point of Care Testing and other clinical services as assigned
 
MAJOR RESPONSIBILITIES:
 
•          Team Leadership 
•        Provides a pharmacy atmosphere and excellent customer service to develop and maintain patient and employee loyalty.
•        Welcomes partnership of management to leverage tools, reports, and processes to exceed sales goals.
•        Leads team to meet and exceed financial goals including payroll, sales, and clinical service expectations.
•        Leads team by modeling, coaching, and maintaining high standards in patient service.
•        Leverages Regional Support Pharmacy Manager, Regional Support Pharmacist, and Senior Regional Support Pharmacist for personal and team development.
•        Organizes and directs workflow.
•        Provides professional services to patients by presenting a positive image of leading by example.
•        Manages pharmacy and clinical services according to payer, state, federal and company regulations to ensure compliance.
•        Maintains clean, neat, and well-organized work area.
•        Develops and follows up on long term care (LTC) business through immediate communication and attention to the needs of the nursing homes, group homes and assisted living residences.
•        Fosters development of the clinical services
•        Supports cross-functional teams by modeling, coaching, and maintaining high standards in patient service.
 
•          Clinical Services 
•        Supports and completes all Pharmaceutical Care responsibilities including but not limited to drug administration, point-of-care testing, patient consultation, disease state management, medication therapy management, administration of immunizations as allowed and other clinical services as assigned
•        Delivers MTM services, including but not limited to, Comprehensive Medication Management,  Comprehensive Medication Reviews (CMR), Targeted Medication Reviews (TMR), Disease State Management, and to assess the appropriateness of all medications, optimize medication therapy, and improve clinical outcomes
•        Utilizes the Joint Commission of Pharmacy Practitioners (JCPP) Pharmacists’ Patient Care Process to consistently deliver patient care and address medication-related needs 
•        Determines appropriate interventions to resolve medication therapy problems, achieve the goals of therapy, and prevent new medication therapy problems
•        Uses evidence-based practices and current clinical guidelines to formulate recommendations and treatment plans
•        Maintains clinical rules and protocols in internal software systems 
•        Individualizes patient care by evaluating  and documenting clinical responses to therapy including indication, efficacy, safety, and adherence 
•        Completes patient counseling in compliance with clinical program protocols and documents any and all interactions with patients and prescribers in the appropriate systems in compliance with protocols
•        Conducts patient-centered adherence services 
•        Communicates clinical recommendations and collaborate with prescribers, interdisciplinary care teams, patients, and/or caregivers to identify and develop patient-specific disease-state therapy goals
•        Coordinates care to meet patient outcomes including, initial referrals, medication review, and selection, assessment of medication delivery system, coordinated care conferences, patient care plans, compliance programs, and discharge of patients from services.
•        Performs CLIA-Waived Point-of-Care Testing per protocol
•        Integrate point-of-care testing (POCT) into patient care including performing CLIA-waived POCT, adhere to established protocols and quality control procedures, interpreting results, document results in accordance with policies and regulations, counseling patients on their health conditions and/or medication management, and collaborating with other health care providers. 
•        Provide oversight to pharmacy technicians or pharmacy interns, whom are delegated point-of-care testing (POCT) responsibilities, as allowed per corresponding state law. 
•        Measures and obtains objective patient information to assist in clinical care plan  
•        Prescribe medication according to Collaborative Practice Agreements
•         Serves as an up-to-date drug information resource to assist all staff members
•        Engages patients through quality interactions and relationship-building.
•        Collaborates with Long Term Care pharmacy teams to deliver and conduct clinical service initiatives 
•        Provides and promotes immunizations in store and off-site as needed and with local employers and long term care facilities 
•         Participates in local health fairs and identifies opportunities to serve as an information resource for the community as directed by the company 
•         Actively contributes to quality improvement initiatives and performance metrics related to patient care and clinical services.
•         Address open quality gaps with patients and work collaboratively with patients towards closure of those gaps 
•         All other duties as assigned
ADDITIONAL REQUIREMENTS: 
•          Working knowledge of third-party pay plans and of various quality measurement entities, such as the Centers for Medicare & Medicaid Services (CMS) and Pharmacy Quality Alliance (PQA)
•          Excellent customer service and communication skills
•          Proficient in the English language
•          Maintain patient confidentiality at all times
•          Attention to detail and ability to follow policies and procedures
•          Ability to use Microsoft Office products and execute basic computer skills.
•          Ability to handle stressful situations and deal successfully with difficult customers
•          Ability to maintain effective working relationships with employees and supervisors
•          Understands laws concerning Continuing Education requirements and takes responsibility to accomplish them
•          Reliable attendance and punctuality in order to fulfill the essential job functions
•          Availability to work evenings, weekends, and holidays when needed
•          Meets productivity and performance expectations 
 
REQUIRED QUALIFICATIONS
EDUCATION
•       B.S. Degree or Pharm.D. in Pharmacy from an accredited educational institution.
EXPERIENCE
•       Three years of experience as a practicing pharmacist
•       Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the ambulatory care patient in collaboration with physicians, nurses, and other healthcare professionals.
 
LICESENSE/CERTIFCIATION
•       Current Minnesota pharmacist license
•       Current or willing to obtain North Dakota, Iowa, and/or Wisconsin pharmacist license 
•       Certificate of training for Immunizations
•       Certification in CPR or BCLS
 
PREFERRED QUALIFICATIONS
EDUCATION
•       PharmD
•       Minimum of two years of professional experience as an Ambulatory Care Pharmacist or, a combination of Residency training, education, and experience that is equivalent to the employment standard listed above and that provides the required knowledge, abilities, and license. 
•       Clinical background in Ambulatory Care 
 
LICESENSE/CERTIFCIATION
•       Board of Pharmacy Specialty (BPS) Certification 
•       Currently licensed or willing to consider licensure as a Registered Pharmacist in multiple states, including all states the organization operates in. Current and unrestricted clinical licensure is a requirement.
