Clinical Pharmacist – Louisiana License Required
Location: Remote with occasional on-site visits
Schedule: Rotating shifts between 8:00 AM – 8:00 PM CST, Monday–Friday (flexibility required; occasional weekends possible)
About The Role
The Clinical Engagement Services Center team delivers a wide range of clinical services, including Adherence, Diabetes Management, Fraud Waste & Abuse, Medication Therapy Management (MTM), Retrospective Drug Utilization Review, and STARS adherence programs. Our team consists of clinical pharmacists, certified pharmacy technicians, and customer service representatives working collaboratively to meet operational and clinical goals.
As a Clinical Pharmacist, you will report to the Operations Team Supervisor and Manager. Your primary responsibility will be conducting telephonic consultations with members to complete comprehensive medication reviews and improve disease state management. You will also document care provided and communicate essential information to members’ primary care providers. Occasionally, you may conduct in-person consultations.

Key Responsibilities
- Engage members telephonically and schedule pharmacist consultations for clinical programs supported by the Engagement Services Center.
- Utilize prescription claims data to generate clinical recommendations per MTM and Retrospective Drug Utilization Review protocols, addressing gaps in care, high-risk medications, adherence, drug interactions, and therapeutic/generic substitutions.
- Assess medication regimens for unnecessary or ineffective medications, inappropriate doses, and adverse drug reactions.
- Provide patient education on medications and disease states.
- Accurately document patient care notes and recommendations in a clear, concise, and professional manner.
- Stay current with emerging clinical trends and guidelines.
- Actively participate in departmental meetings to improve programs and processes.
- Meet productivity and performance expectations set by management.
- Support special projects as needed.
- Perform other duties as assigned.
- Bachelor’s Degree in Pharmacy or PharmD.
- Current, unrestricted Pharmacist license in Louisiana.
- Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services:
- Medication Therapy Review (Comprehensive & Targeted)
- Personal Medication List (PML)
- Medication-Related Action Plan (MAP)
- Interventions and Referrals
- Documentation and Follow-up
- Ability to work independently in a remote, cross-functional team environment.
- Proficiency in navigating MTM platforms and multitasking across multiple databases/applications.
- Competent use of technology and computer systems.
- Flexibility to work rotating shifts between 8:00 AM – 8:00 PM CST, Monday–Friday; occasional extended hours/weekends as needed.
- PharmD.
- Bilingual (medical terminology) in Cantonese, Korean, Mandarin, Russian, Spanish, or Vietnamese.
- 2 years of pharmacy experience in retail/community pharmacy or PBM (new graduates with experiential rotations considered).
- Additional pharmacist licenses in AL, ID, KY, MD, MN, OR, TN, or VT.
- Prior call center experience.
- ACPE MTM Specialist certification.
- Certified Geriatric Specialist.
- Board Certified Pharmacist.
Salary : $50 - $60
