Description: The Foundational Community Support (FCS) Specialist provides case management for supportive employment and supportive housing to empower individuals facing various challenges to secure stable housing and gain meaningful employment. They provide personalized assistance, guidance, and resources to help clients achieve independence and improve their quality of life.Job Duties/ResponsibilitiesAssessment and Goal Setting:Conduct comprehensive assessments of clients to understand their unique needs, strengths, and goals.Collaborate with clients to develop individualized plans focused on obtaining stable housing and meaningful employment.Housing Support:Assist clients in searching for safe and affordable housing options, including helping with housing applications, navigating housing programs, and securing subsidies if needed.Provide ongoing support to help clients maintain their housing stability, including addressing issues related to rent, utilities, and property maintenance.Employment Support:Conduct job readiness assessments to identify clients' skills, interests, and employment barriers.Assist clients in job searching, resume development, interview preparation, and job application processes.Collaborate with local employers and organizations to create job opportunities for clients.Benefits and Financial Literacy:Help clients access and understand government benefits, such as disability benefits, Medicaid, or food assistance programs.Provide financial literacy education to help clients manage their finances, budget effectively, and reduce financial stress.Life Skills Training:Offer training and coaching on life skills, including time management, problem-solving, communication, and conflict resolution.Advocacy and Community Integration:Advocate for clients' rights and needs within the community and with service providers.Encourage clients to engage in community activities and support networks to build social connections and a sense of belonging.Crisis Intervention:Provide crisis intervention support and guidance to clients during difficult situations, such as housing instability or job loss.Documentation and Reporting:Maintain accurate and up-to-date client records, progress notes, and case files.Generate reports and documentation required by funding agencies and regulatory bodies.Maintain client’s eligibility status’ as well as TAPS qualified reimbursement and/or disbursement of funds.Transportation and Community Services:Transport clients to appointments, job interviews, housing viewings, and other community-based activities as neededProvide services in clients' homes and various community settingsTravel throughout the service area to meet clients where they areOther Related Duties:As a mandated reporter, follows all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect.Actively participates in staff meetings and multi-disciplinary meetings with other team members.Actively participate in team and/or individual supervision as stated in agency policy.Identify and address members’ barriers to independence and recovery.Facilitate learning opportunities by teaching life skills classes and leading activities.Complete all required documentation.Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of Three Rivers Therapy client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA).Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by manager.Perform as a team member to assure that productivity outcome measures are achieved.Requirements:To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job. Education/Experience:?to perform this job successfully, an individual must have and?possess Peer Support Certification. Minimum Qualifications:Certification as a Peer Support must be obtained within three months of hire.Operate from a strengths-based perspective.Possess good written and verbal communication skills and excellent documentation skills.Ability to work independently and as a team member.Working knowledge of community resourcesRepresent the agency in a professionally within the communityDevelop collaborative community relationships.Basic computer proficiency?Preferred Skills:Bachelor's degree in a related field (e.g., social work, psychology, counseling) or equivalent experience.
