Position Title: HCBS Case Manager
Location: Franklin, GA.
Position Summary
The HCBS Case Manager is responsible for conducting in-home assessments and providing case management services to elder clients referred through the Aging and Disability Resource Center (ADRC) across our 10-county region and other applicable community referral sources. This position works closely with clients, family members, and caregivers to identify resources that support independence and safety in the home.
Primary Responsibilities
- Conduct in-home assessments and provide ongoing case management services
- Collaborate with clients, families, and caregivers to identify and coordinate supportive services
- Develop and implement individualized care plans
- Consult with supervisor on complex or problematic cases
Summary of Duties
- Conduct initial in-home assessments and identify service needs
- Develop care plans and coordinate services to sustain independence
- Collect and record client and statistical data in the approved state reporting system
- Refer clients to appropriate community resources
- Follow up with clients, caregivers, and service providers as needed
- Maintain client files in accordance with Division of Aging Services and Area Agency on Aging requirements
- Maintain assigned caseload in accordance with agency policy
- Complete required reports and reassessments timely
- Refer clients to Adult Protective Services (APS) when appropriate
- Participate in case conferences with supervisors and agency staff
- Oversee service delivery effectiveness
- Attend required meetings and trainings
- Perform other duties as assigned
Time Allocation
- Face-to-face client contact: 50%
- Assessment write-ups: 20%
- Reassessments: 10%
- Resource coordination for clients: 10%
- Evidence-based trainings: 10%
Qualifications & Requirements
- Bachelor’s degree from an accredited four-year college or university in social work, social services, health, nursing, or a closely related field
- Background in gerontology, long-term care, or community-based service delivery preferred
- Equivalent combination of education and experience may be considered
- Ability to travel up to 50% of the time within a 10-county region
- Valid driver’s license and reliable transportation required
- Other Requirements:
Pre-employment drug screen; criminal background check; proof of a valid driver’s license; conformity to Three Rivers Regional Commission standards of conduct for employees; professional business appearance and demeanor; appropriate language skills to communicate with a variety of business organizations and people, means and ability to travel throughout a multi-county area and some overnight travel, good work history. Must be able and willing to work a flexible schedule including evenings if necessary. Must be able to lift at least thirty pounds.
Preferred Qualifications:
Working experience in maintaining a Case Management caseload in a social services field;
