The LPN works in collaboration and continuous partnership to provide expert individualized high risk care to patients and their families. The LPN will support timely scheduling for diagnostic work-up, results reporting and communication between the patient and the multidisciplinary team ensuring the patient receives necessary referrals and follow ndash;up. The LPN serves as an advocate for the patient, promoting timely, competent, compassionate care and enhances the physician/patient relationship improving the experience of the patient.

• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
• Collaborates and continuously partners with high risk patients and their family, clinic/hospital/specialty providers and staff in a team approach.
• Prepares patients for what to expect throughout the care continuum.
• Reports blood results in collaboration with OB and MFM providers
• Promotes timely access to appropriate care such as helping to schedule appointments.
• Supports communication between the care team and the patient, facilitates screening and addresses barriers to care and documents interactions in the electronic health record
• Attend and support MOAT/SOAT

Education: Graduate of a school of Practical Nursing AND Experience: 2-3 yearsrsquo; experience in health care preferred; experience in OB preferred

Minimum Certifications, Registration or License Requirements: LPN licensure in PA

Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
