What are the responsibilities and job description for the Care Coordinator, Clinic position at Thibodaux Regional Health System?
Responsible for coordination of all patient needs for biologic and infusion medication management.
QUALIFICATIONS:
Educational and/or Professional Qualifications:
Graduate of a LPN school of nursing required
Previous Experience:
- Minimum of three years of experience in a health care setting required.
- Knowledge of coordination of medication management and reimbursement and regulatory
Licensure, Certification, Registration, etc.:
Current licensure LPN in the State of Louisiana required.
