Direct Care Worker

Job Summary

Responsibilities

• Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor residents' health status by observing patient behavior and vital signs such as blood pressure, pulse, and respiration rates
• Administer medications accurately following care plans and HIPAA confidentiality standards
• Support residents during meal preparation and feeding to ensure proper nutrition and hydration
• Operate assistive devices such as Hoyer lifts and ensure proper use of safety equipment for patient transfers
• Maintain detailed documentation of care provided, patient observations, and any changes in condition in compliance with healthcare regulations
• Implement individualized care plans tailored to each resident's needs, including memory care for individuals with dementia or Alzheimer's disease
• Engage residents in social activities and provide companionship to promote mental well-being
• Perform light cleaning duties to uphold hygiene standards within the living environment
• Collaborate with healthcare professionals, social workers, and family members to deliver comprehensive care
• Follow all safety protocols including CPR, first aid procedures, and infection control practices to ensure resident safety at all times

Requirements

• Prior experience working in group homes, assisted living facilities, nursing homes, or home & community care settings is highly preferred
• Knowledge of patient observation techniques and vital signs monitoring
• Experience with Alzheimer's care, dementia care, memory support, or working with individuals with developmental disabilities or autism is desirable
• Certification in CPR and First Aid required; additional certifications such as HIPAA training are a plus
• Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
• Strong communication skills to effectively interact with residents, families, and team members
• Demonstrated patience, compassion, and dedication to providing high-quality caregiving services
• Ability to work flexible hours including evenings, weekends, or overnight shifts as needed
• Physical stamina to stand for extended periods and assist residents with mobility challenges
