Role: Travel Home Health Physical Therapist, responsible for providing patient care in Medford, Oregon.
Key Responsibilities: Conduct physical therapy visits, start of care assessments, evaluations, recertifications, return of care, and supervisory visits within the coverage area.
Qualifications & Skills: Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited institution, active State License, and home health experience. BLS certification may be required.
Key Duties: Deliver physical therapy services, manage patient recovery, and support chronic condition management, ensuring quality care in a home health setting.
