- Conduct weekly in-home nursing visits, including physical assessments, medication management, home safety checks, and overseeing caregivers.
- Develop and evaluate care plans, coordinate with clients and healthcare teams, and ensure quality of care.
- Serve as a clinical resource on-site, support staffing and scheduling, and adhere to agency policies and scope of practice.
- Participate in staff training, review clinical documentation, and contribute to performance improvement initiatives.
- Assist with client and physician engagement and perform supervisory visits as needed.
- Graduate of an approved nursing program with an unrestricted RN license.
- BSN preferred, with at least two years of nursing experience, including one in home care.
- Current CPR certification and supervisory or case management experience preferred.
- Strong clinical assessment, communication, and organizational skills.
- Ability to work independently, collaborate with teams, and manage multiple priorities.
