Pay Rate:
Range $2320 to $2600 weekly Gross pay based on a 40 hour work week (Includes non-taxable
travel/housing and M&I stipend available for qualified/eligible travelers)
Unit: Hospice in a home health setting
Shift: 8 Hours; Days, 8.00 am - 5.00 pm
Schedule: 40 hours/week guaranteed, 8-hour shifts, 5 days a week, Monday through Friday. Some weekends and On-call may be required.
- Registered Nurse: 1 years - Required
- Hospice Nursing and Case Management experience in a Home Health setting: Recent/Current, 1 years – Required
- Acute and Med Surg experience - Highly preferred
- Wellsky Consolo charting experience - Highly preferred
- Valid Driver's License and Active Auto Insurance - Required
Education, License, and Certification Requirements:
- ASN or an equivalent – Required
- BSN or an equivalent – Highly
preferred
- RN License for State of WA or Compact - Required
- RN License for State of OR - Strongly encouraged (Can apply upon offer if required)
- AHA BLS Certification - Required
- Valid Driver's License and Active Auto Insurance - Required
Job Description
- Manage
a caseload of 15 patients, with 4-5 visits per day, depending on the area and acuity level of patients, and complete charting on the same day.
- In-charge
of providing care, comfort, and support to assigned patient case load, navigating the difficult times at the end of their lives.
- Develops
plans for individual patients to ensure needs are met, performs regular checkups, and communicates with family members as needed.
- As a
member of the interdisciplinary team, the RN will provide skilled nursing
assessments, planning, and care in order to maximize the comfort and health of
both patients and their loved ones/caregivers
- The RN will provide holistic care to patients and their families, incorporating nursing care that reflects observations, actions, and plans in accordance with the IDT Plan of
Care.
- Must
be a team player who supports an interdisciplinary team concept of caring for
patients and loved ones/caregivers.
