Physical Therapist (Sign-On Bonus)

Role: Licensed Physical Therapist providing one-on-one geriatric care in private homes and senior living communities, focusing on improving mobility, independence, and quality of life.

Responsibilities

• Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy sessions.
• Support patient wellness, functional independence, and aging in place through in-person and virtual care.
• Document patient progress accurately in EMR systems and collaborate with interdisciplinary teams.
• Educate patients, caregivers, and staff on safety and functional strategies.
• Incorporate telehealth when appropriate to enhance care continuity and access.
  
  

Qualifications & Skills

• Valid PT license or eligibility in the state.
• Degree from an accredited physical therapy program.
• Proficiency in basic computer use and EMR documentation.
• Current CPR certification.
• Ability to perform physical tasks including lifting, repositioning, and assisting with patient mobility.
