Role Overview: Provide in-home physical therapy services focused on assessing and treating speech, language, communication, swallowing, and cognitive impairments. Develop and implement personalized treatment plans, monitor patient progress, and collaborate with healthcare team members.

Key Responsibilities

• Perform patient evaluations using diagnostic tests to determine functional abilities.
• Assist in developing and executing therapy plans in coordination with physicians.
• Treat communication and swallowing disorders, helping patients express themselves effectively.
• Document patient responses and report changes to supervising healthcare professionals.

Qualifications & Skills

• Licensed in Physical Therapy in the state of residence.
• Current CPR certification.
• Valid driver's license, reliable transportation, or access to public transit.
• Willingness to work a weekend rotation.
