- Providing care as per state nursing practice acts and implementing care plans.
- Documenting accurately and timely, assessing patient needs, and educating patients and families.
- Coordinating home health services and efficiently using resources.
- Communicating effectively with team members, preparing clinical notes, and reporting concerns.
- Performing duties assigned by the registered nurse.
- Graduate of an accredited practical or vocational nursing program.
- At least three years of nursing experience, including community or medical/surgical care.
- Current licensure as a practical or vocational nurse.
- Strong communication, organization, and problem-solving skills.
- Valid driver's license, insured vehicle, and current CPR certification.
- Ability to stand, walk, lift, and reposition patients and supplies.
- Good visual and auditory acuity to perform nursing duties.
- Adherence to professional standards and compliance policies.
