LPN Med Care Manager

Role Overview

The Licensed Practical Nurse (LPN) Medication Nurse provides licensed nursing support to senior residents, ensuring safe medication administration, health monitoring, and accurate clinical documentation.

Key Responsibilities

• Administer medications and treatments according to physician orders
• Follow the Six Rights of Medication Administration
• Document accurately in MAR/TAR in real-time
• Monitor symptoms, report adverse reactions, and support symptom relief
• Conduct Service & Health Updates (SHUs) and transcribe physician orders
• Observe and report physical, emotional, or cognitive changes
• Maintain medication carts, complete narcotic counts, and ensure compliance
  
  

Qualifications & Skills

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation skills
• Preferred experience in senior care or medication administration
