Seeking a Licensed Practical Nurse (LPN) to provide focused, one-on-one pediatric home health care. You will support pediatric patients with skilled nursing needs, working closely with a registered nurse and clinical leadership to ensure consistent, safe, and effective care.

Seeking a Licensed Practical Nurse (LPN) to provide focused, one-on-one pediatric home health care. You will support pediatric patients with skilled nursing needs, working closely with a registered nurse and clinical leadership to ensure consistent, safe, and effective care.
Responsibilities
- Deliver individualized care in the home setting
- Follow care plans with RN oversight
- Maintain safety protocols and proper documentation
- Collaborate with the clinical team for optimal patient outcomes
- Valid New Jersey LPN or Multistate License
- Recent physical exam and PPD or Chest X-Ray
- Valid BLS CPR card (in-person training)
- Valid driver’s license
- Experience with G-tube, trach, vent preferred or willingness to train
