Responsibilities
- Deliver high-quality clinical services following state laws and care plans
- Accurately document patient visits and activities
- Participate in care coordination, case conferences, and communication efforts
- Adapt to various home environments and patient needs
- Current, unrestricted LPN license in the practice state
- Valid driver’s license, vehicle insurance, or access to transportation
- Preferred: CPR certification, home care experience, strong organizational and communication skills, ability to work independently, and manage multiple tasks
