A Licensed Practical Nurse (LPN) provides compassionate, skilled nursing care to geriatric clients in their homes, working as part of a multidisciplinary team to support clients' ability to remain at home.
Key Responsibilities
- Conduct home visits to assess and care for clients in designated areas.
- Administer medication, wound care, treatments, and procedures.
- Monitor client conditions, report changes, and follow physicians' orders.
- Perform client assessments and coordinate case management.
- Document care activities accurately using electronic medical records.
- Current North Carolina LPN license.
- Minimum of one year recent, verifiable LPN experience.
- Graduation from an accredited nursing program.
- Ability to work independently, manage time effectively, and communicate well.
- Strong computer skills; EMR experience preferred.
- Willingness to travel to client locations.
