Licensed Practical Nurse In Home Health
Role: Provide quality patient care in patients' homes, performing skilled clinical services within the scope of practice and according to the plan of care. Collaborate with healthcare team members, document visits thoroughly, and participate in care coordination activities.
Responsibilities: Deliver high-quality clinical services, ensure proper documentation, and communicate effectively with patients, families, and healthcare providers. Follow all infection control procedures and contribute to care planning.
Qualifications: Current LPN licensure, CPR certification, valid driver’s license, dependable transportation, and ability to work flexible hours. Must be able to work in diverse home environments. Minimum one year of LPN experience preferred.
Skills: Ability to manage multiple tasks independently, strong communication, organizational skills, and adaptability to various home settings. Experience in home care is advantageous.
